The results were published in the journal Nature Medicine.&nbsp;<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Ali Aminian, M.D., director of Cleveland Clinic&#8217;s Bariatric and Metabolic Institute and principal investigator of the study, said the results show that bariatric surgery should be considered as a treatment option for patients with cirrhosis and obesity. &#8220;Bariatric surgery was associated with a 72% lower risk of developing serious liver disease complications and an 80% lower risk of progression to the decompensated stage among patients with compensated cirrhosis and obesity.&#8221;&nbsp;<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Obesity and diabetes are the main causes of metabolic dysfunction-associated steatohepatitis (MASH), the most common form of chronic liver disease in the United States. The build-up of fat inside liver cells triggers a series of events that ultimately lead to scarring of the liver. Around 20% of people with MASH may progress to advanced-stage liver scarring, known as cirrhosis. An estimated three million people in the United States have MASH-related cirrhosis.&nbsp;<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Cirrhosis is classified into two distinct stages: compensated and decompensated. In the compensated stage, despite considerable damage, the liver retains enough residual function to meet the body&#8217;s needs and patients may appear relatively healthy. In the decompensated stage, severe and potentially fatal complications occur that reflect the liver&#8217;s inability to sustain vital functions, and liver transplantation becomes essential for survival.&nbsp;<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">&nbsp;&#8220;Patients with MASH-related cirrhosis have extremely limited treatment options. Currently, no therapeutic intervention has been shown to be effective in reducing the risk of severe liver complications in this patient population,&#8221; said Sobia Laique, M.D., a transplant hepatologist and co-investigator of the study at Cleveland Clinic. &#8220;This underlines a critical unmet need for the development of effective therapies specifically targeting patients with compensated MASH-related cirrhosis.&#8221;&nbsp;<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">The aim of the SPECCIAL study (Surgical Procedures Eliminate Compensated Cirrhosis In Advancement Long-term) was to examine the long-term outcomes of bariatric surgery on the risk of developing major liver complications in patients with obesity and compensated MASH-related cirrhosis, compared with non-surgical management.&nbsp;<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">A group of 62 Cleveland Clinic patients with compensated MASH-related cirrhosis and obesity who underwent bariatric surgery were compared with a control group of 106 non-surgical patients and followed for 15 years. The study participants had similar characteristics, such as the severity of liver disease on their initial liver biopsy.&nbsp;<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Fifteen years after enrolment, the results show that 20.9% of the surgical group and 46.4% of the non-surgical group developed one of the major complications of liver disease, including liver cancer and death.&nbsp;<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Over 15 years, 15.6% of the surgical group and 30.7% of the non-surgical group progressed from compensated cirrhosis to the decompensated stage. At 15 years, patients in the bariatric surgery group had lost 26.6% (31.6 kg) of their weight and patients in the non-surgical control group had lost 9.8% (10.7 kg) of their weight.&nbsp;<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Steven Nissen, M.D., Chief Academic Officer of Cleveland Clinic&#8217;s Heart, Vascular &amp; Thoracic Institute and senior investigator of the study, said: &#8220;Currently, lifestyle intervention is the only therapeutic recommendation for compensated MASH-related cirrhosis. However, lifestyle changes alone rarely provide the weight loss and metabolic changes needed to reduce the risk of liver complications in this patient population. The SPECCIAL study shows that bariatric surgery is an effective treatment that can influence the trajectory of cirrhosis progression in selected patients.&#8221;&nbsp;<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">The authors note that the SPECCIAL study is the first to examine long-term clinical outcomes after bariatric surgery in patients with MASH-related cirrhosis.
