The paper was published in&nbsp;<a href=\"https:\/\/journals.lww.com\/annalsofsurgery\/fulltext\/2024\/09000\/renoprotective_effects_of_metabolic_surgery_versus.7.aspx\" target=\"_blank\" rel=\"noreferrer noopener\"><em>Annals of Surgery<\/em>.<\/a>&nbsp;<\/p>\n\n\n\n<p class=\"wp-block-paragraph\"><mark class=\"has-inline-color has-vivid-cyan-blue-color\">According to the Centers for Disease Control and Prevention (CDC)<\/mark>, more than <a href=\"https:\/\/www.cdc.gov\/kidney-disease\/php\/data-research\/index.html#:~:text=More%20than%201%20in%207,are%20estimated%20to%20have%20CKD.&amp;text=As%20many%20as%209%20in,not%20know%20they%20have%20CKD.\" target=\"_blank\" rel=\"noreferrer noopener\">1 in 7<\/a> adults in the United States are estimated to have chronic kidney disease, a condition that can lead to kidney failure. Chronic kidney disease is a long-term condition that impairs the kidneys&#8217; ability to filter waste from the blood. As the kidneys begin to fail, this waste builds up in the body. In some cases, patients need dialysis, a procedure that filters the blood through a machine. <\/p>\n\n\n\n<p class=\"wp-block-paragraph\"><mark class=\"has-inline-color has-vivid-cyan-blue-color\">Ali Aminian, M.D.<\/mark>, lead author of the study and director of Cleveland Clinic&#8217;s Bariatric and Metabolic Institute, said: &#8220;Our study shows that bariatric surgery can protect the kidneys and prevent kidney function from getting worse. In patients with obesity and type 2 diabetes who already have established chronic kidney disease, bariatric surgery can change the course of the disease.&#8221; <\/p>\n\n\n\n<p class=\"wp-block-paragraph\">The study included 425 adult patients (aged 18 to 75) with type 2 diabetes, obesity and stage 3 or 4 chronic kidney disease. Of the participants, 183 underwent bariatric surgery, while 242 were treated with GLP-1 receptor agonist medications.&nbsp;<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">The study patients underwent bariatric surgery between 2010 and 2017, or continuously received GLP-1 medications that were FDA-approved and available during the research period. In the non-surgical group, the most common GLP-1 medications were liraglutide and exenatide. During follow-up, 20% of patients in the non-surgical group received newer GLP-1 medications such as semaglutide (Ozempic) or tirzepatide (Mounjaro) at some point.&nbsp;<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">The researchers found that bariatric surgery could protect kidney function and reduce patients&#8217; risk of kidney failure or death. Specifically, the findings showed that bariatric surgery was associated with a 60% lower risk of kidney disease progression and a 44% lower risk of developing kidney failure or dying, compared with the non-surgical group.&nbsp;<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Over the eight years of the study, it was also found&nbsp;that 22% of participants in the surgical group experienced kidney disease progression, compared with 45% of the non-surgical group. In addition, 24% of the surgical group developed kidney failure or died within the seven-year study period, compared with 44% of the other group.&nbsp;<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Participants who underwent bariatric surgery experienced greater weight loss and better control of their diabetes. They also needed fewer medications to control their diabetes, high blood pressure and high cholesterol.&nbsp;<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">&#8220;Despite its protective benefits for the kidneys, bariatric surgery is underused in clinical practice,&#8221; said Dr. Aminian. &#8220;Current guidelines for managing chronic kidney disease recommend maintaining a healthy weight through lifestyle changes, but they have not considered this surgery as an option.&#8221;&nbsp;<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Jonathan Taliercio, D.O., a nephrologist at Cleveland Clinic and co-author of the study, added: &#8220;These figures are promising and show that bariatric surgery should be considered as a treatment option for patients with chronic kidney disease and obesity.&#8221;&nbsp;<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Dr. Aminian noted that GLP-1 receptor agonist medications are evolving rapidly, and their role in combination with bariatric surgery requires further research.&nbsp;<\/p>\n\n\n\n<p class=\"wp-block-paragraph\"><strong>About Cleveland Clinic<\/strong><strong><\/strong><\/p>\n\n\n\n<p class=\"wp-block-paragraph\"><a href=\"https:\/\/my.clevelandclinic.org\/\">Cleveland Clinic<\/a> is a nonprofit multispecialty academic medical centre that integrates clinical and hospital care with research and education.
